INSURANCE

Bass Lake Family Eye Care accepts most forms of insurance.  If you have any questions about what your insurance covers, or if we are in-network, we recommend that you call the number on the back of your insurance card.

Insurances we accept:  VSP, EyeMed Vision Care, NVA (National Vision Administrators), Blue Cross Blue Shield, PreferredOne, Medica, HealthPartners, CIGNA, CIGNA Vision, United Healthcare, Aetna, Patient Choice, Regional Care, Inc., Definity Health, UCare (services only), Medical Assistance (services only)

We now accept Humana Vision insurance!

We are currently not providers for United Healthcare Vision, Davis Vision, or Spectera.

VSP (VISION SERVICE PLAN)
We are one of only a few local providers for VSP. Though there are several different VSP plans, most offer coverage for glasses or contact lenses as well as for a routine exam. Please call us and we can let you know your individual coverage. 

EYEMED
EyeMed Vision Care is held by several local companies. We are providers for most plans. Many plans offer coverage for routine exams, and most offer coverage or discounts for eyeglasses or contact lenses. Please contact us and we can determine your eligibility and let you know your coverage.
